We are thrilled to welcome you to our studio and would like to thank you for considering us for your prosthetic wig needs. In order to set up an appointment, please email us the following information:

- Your full name, address, phone number, and reason for coming in

- 3 recent photos of yourself (With your hair if possible)

- A prescription from your doctor (if you have one) 

(The prescription would allow you to submit our receipt, which has the procedure codes, to your insurance carrier and if you are covered, they would reimburse you. We do not work with insurance companies).

 -  Please shampoo your hair on the day of your appointment 

 - We request a 24-hour cancellation notice if necessary.

 This information will allow us to create your file before you come in. Once we receive your email, we will give you a call and set up your appointment.

We provide parking

 Please note that masks are now optional and we kindly request that you bring no more than one person with you for your appointment.
